Case Management

Case Management is a collaborative person-centered process that includes intake, needs assessment, service planning, monitoring and evaluation. This process incorporates input from the member, caregiver, and our team.

 Our case management team consists of a Registered Nurse (RN) and a case manager. The program is designed to support the enrolled member and the caregiver by giving them the tools needed to help the member remain safely in the community. During the assessment and care planning process we are able to identify the member’s strengths and challenges as well as develop goals that will help to increase their level of functioning and overall wellbeing.

The team also assesses the caregiver to identify strengths, weaknesses and potential challenges that might affect the placement.

Case Management Provides:

  • Assistance with completing application for Department of Transitional Assistance, MassHealth, and Department of Developmental Services
  • Connection to Adult Day Health, Day Habilitation Centers, and mental health services
  • Connection to medical provider such as PCPs, dentists, and psychiatrist
  • Help finding full-time caregivers and alternate caregivers.
  • Information on educational resources and services such as GED, Special Olympics and Caregiver Support Groups
  • Monthly in home visit with the member and caregiver.
