
Case Management is a collaborative person-centered process
that includes intake, needs assessment, service planning, monitoring and
evaluation. This process incorporates input from the member, caregiver, and our
team.
Our case management
team consists of a Registered Nurse (RN) and a case manager. The program is
designed to support the enrolled member and the caregiver by giving them the
tools needed to help the member remain safely in the community. During the
assessment and care planning process we are able to identify the member’s
strengths and challenges as well as develop goals that will help to increase their
level of functioning and overall wellbeing.
The team also assesses the caregiver to identify strengths,
weaknesses and potential challenges that might affect the placement.
