Medical Oversight

Our dedicated team of Registered Nurses(RNs) completes needs assessments, develops, and implements individualized Care Plans with assistance from the members, caregivers and case managers. The Registered Nurse conducts a home visit with each member at least once per month or as needed based on the member’s need.

The RN is also responsible for assessing the caregiver to identify strengths and areas of need in order to make referrals to connect the family to the resources needed to keep the member safe in the community.

Medical Oversight Includes:

  • Assistance with understanding the member’s diagnosis and medications.
  • Assistance with medication reconciliation.
  • Connection to medical providers such as PCP, dentist, psychiatrist, and other specialists.
  • Help finding full-time caregivers and alternate caregivers.
  • Information on educational resources and services such as Caregiver Support Groups and useful websites for medical information.
  • Recommendations for medical concerns identified during visits.
  • Monthly in home visit with the member and caregiver
